Duties Performed:
Perform medication reconciliation for newly admitted patients. Perform drug distribution, which includes entering physician orders into the patient profile using WORx pharmacy software, medication order review for patient safety and medication appropriateness; Pyxis refill verification; filling discharge, authorized-leave, and self-medication prescriptions. Must have the ability to interpret medical and drug literature. Must accurately use pharmacy technology such as WORx, Cache, Pyxis, Micromedex, UpToDate, MediMAR, Medline, and Cerner. Develop and maintain procedures to improve the efficiency and accuracy of medication distribution and administration. Facilitate efficient medication delivery and appropriate scheduling of medications. Oversee pharmacy technicians and verify the accuracy of their work. Provide patient medication counseling at the time of patient's discharge. Perform duties of the on-call pharmacist OR perform pharmacist duties on the weekend on a rotating basis. As a member of an interdisciplinary healthcare team using advanced skills obtained from training and at WSH, provides comprehensive pharmacy services to WSH and CSTC patients receiving medication therapy. Ensure appropriate drug therapy including indication, dose, drug interactions and drug allergies. Evaluate pharmacotherapeutic regimens for effectiveness, adverse drug events, and patient side effects and intervene as needed This position may work under an approved collaborative drug therapy agreement (CDTA). Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations. Provide drug information and education to medical teams, nursing, and dietician staff. Must accurately use pharmacy technology such as WORx, Cache, Pyxis, Micromedex, UpToDate, MediMAR, Medline, and Cerner. Must have the ability to interpret medical and drug literature. Participate on ward rounds and/or treatment conferences with physicians and other healthcare professionals on assigned wards. Monitor patient medications, review new and changed medication orders, perform chart reviews, and monitor adverse reaction and laboratory monitoring. Assess medication toxicity as appropriate and recommend management solutions. Consult with physicians and nurses regarding drug therapy safety issues. Provide comprehensive written and/or oral medication information and patient consultation to nursing and medical staff; including literature search and critical evaluation in the areas of forensic psychiatry, geriatrics, diabetes, anticoagulation, etc. May participate in a clinical team such as the Anticoagulation Team, Diabetes Care Team, Clozapine Safety Monitoring, Pain Management Team, etc. Perform assigned Drug Use Evaluation (DUE) including safety and cost assessments and report to the Pharmacy and Therapeutics P&T Committee. Review assigned formulary sections annually for new safety, efficacy, and/or cost information and present to the Pharmacy and Therapeutics (P&T) Committee. Research new or alternative treatments specifically used to treat psychiatric disorders. Evaluate patient monitoring and information systems related to the optimal care of patients with psychiatric disorders. May provide preceptorship of pharmacy residents and student pharmacists Provide education to medical residents, nursing staff, pharmacy staff, and medical students. Present at conferences or committee meetings to multidisciplinary staff, as requested Develop and provide drug in-services, as requested Lead ward-based medication education to patients and staff. Teach psychiatric and diabetes drug information to nursing staff during their new employee orientation. Implement and maintain departmental policies and procedures related to the safe and effective use of medications; usually accomplished as part of a committee. Develop and maintain drug therapy monitoring systems for patients receiving psychotropic drugs; usually accomplished as part of a committee. Establish and maintain efficient lines of communication between pharmacists, physicians, nurses, and hospital staff. Participate in the development and implementation of quality assurance programs for pharmacy services, including, but not limited to, prevention of medication errors.

Graduation from an accredited college of pharmacy with a Doctor of Pharmacy (Pharm D) degree. Valid Washington State license to practice pharmacy or eligibility to obtain licensure prior to employment. Permanent appointment will be contingent upon obtaining a valid Washington State license to practice pharmacy within 90 days following appointment.
