Job Summary: Implement and administer necessary and appropriate transition services for students with special needs who have socially graduated and have chosen to continue accessing secondary services. Typically these are students 18-21 years of age. Provide oversight of transition services and IEP compliance issues for high school students with special needs. Requirements of the Transition Specialist position are governed by the transition regulations found in IDEA 2004.

Essential Duties and Responsibilities:

Provide case management for 18-21 year old students with special needs and monitor progress toward post-secondary goals and objectives. Serve as liaison among adult service providers, students, families and SPED staff. Provide consultation services for students, families and APED staff as necessary. Collaborate with various community agencies and adult services providers to provide services and support for students and families. Attend all senior IEP meetings at Harrison High School, Sierra High School, The New Horizons School and James Irvin High School to assist in the development of students' post-secondary pursuits. Assist and/or train secondary SPED staff in IDEA 2004 compliance issues and transition best practices. Assist the Director and/or Coordinator of Special Programs in completing any departmental assignments of importance. Participate on the Pikes Peak Interagency Transition Team to gather necessary resources and information to share with secondary SPED staff. As a member of the Pikes Peak Interagency Transition Team represent HSD2 by contributing to community activities and issues that are of significance to students with disabilities. Participate in continuing education and professional growth activities related to transition. Perform other duties as may be assigned by supervisor/administrator. Education and Training:

Bachelor's Degree. Special experience and training as determined by content area. Skills, Knowledge and Equipment :

In-depth knowledge of content area. Good organization and communication skills. Ability to communicate effectively with stakeholders within the school environment. Knowledge of classroom management and conflict resolution. Knowledge of and experience in Professional Learning Communities. License, Certificates, and Registrations:

Valid Colorado Teaching License with Special Education endorsement. Criminal background check and District fingerprinting required. Judgement and Decision Making :

Work is assigned by self or building administrator. This position requires application of position knowledge to effectively and efficiently instruct content area. Most decisions are made immediately without time for collaboration; supervisor is involved only in major decisions. Safety To Self and Others :

Low exposure to self due to repetitive motion or lifting. Must not pose a direct threat to the health or safety of him/her or others with or without reasonable accommodation. Must be able to attend to the duties of the position for the duration of the assignment.

Physical Demands:

The work is performed in schools and is mostly sedentary with periods of light physical activity. Typical positions require employees to walk or stand for long periods; routinely lift and carry up to 30 pounds, occasionally lift and carry up to 50 pounds; bend, kneel and crouch, reach, hold, grasp and turn objects, use fingers to operate computer keyboards. The work requires the ability to speak normally and to use normal or aided vision and hearing. Work Environment :

While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually low. Mental Functions :

While performing the duties of this job, the employee is regularly required to instruct, communicate and use interpersonal skills; frequently required to compare, analyze, compute, synthesize, evaluate, compile and negotiate.
