Regional Social Worker
The LCSW or LMSW in this role will be responsible for social support services to members/families' regional social service needs and supporting regional and specialties in the Region. Activities include planning and coordinating the social services and community resources necessary to respond to an individual member's health care needs to ensure continuity of care and the most efficient and effective use of resources. In addition, responsibilities include interacting with physicians, members/families, complex case managers, home care reviewers, inpatient care coordinators, medical office staff, and other Kaiser Permanente staff, education of providers, member/family and others, and promotion of a high level of member satisfaction.
This position is located at our TownPark Medical Office in Kennesaw, GA and will mainly support Palliative Care. Monday - Friday (no weekends) and mostly in office with occasional virtual opportunities. There is also occasional coverage for holidays.
Major Responsibilities/Essential Functions Performs functional, psychosocial, financial assessments, social determinates of health screening of the member/family. Plans interventions related to the social service needs based on the assessment. Life Care Planning Interventions Communicates social services interventions to the member/family and makes changes as appropriate. Identifies, integrates, and coordinates Kaiser Health Plan and community health care resources and services for members and works in conjunction with Community Medical Services as appropriate. Continuously coordinates, monitors, tracks, and elevates all social service rendered to ensure that quality care is being delivered in the most appropriate setting. Maintains contact with patients and their families as appropriate, including the provision of education when needed. Maintains effective interaction/communication with members of the medical staff, nursing staff, inpatient care coordinators, the SNF coordinator, home care review team, complex case managers, general reviewers, referral coordinators, Kaiser Permanente medical offices, other departments within the plan, hospitals, SNFs and other community resources to facilitate the process of meeting the member's social service needs and to provide continuity of care Minimum Education
Current unrestricted licensure in the state of Georgia as a Licensed Master's Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) Minimum Work Experience and Qualifications
Experience with managed health care delivery including Medicare At least two (2) years of post-graduate experience in a health care setting Knowledge of community resources required Working knowledge of all relevant federal, state, local and regulatory requirements including Medicare/HCFA Functional knowledge of computers Demonstrated customer service aptitude on a Customer Service Assessment Preferred Work Experience and Qualifications
Experience in discharge planning and/or quality improvement in a managed care setting preferred Required Licensure, Certification, Registration or Designation
Current unrestricted licensure in the state of Georgia as a Licensed Master's Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW)
