The Clinical Dietitian works with both the inpatient population as well as the outpatient population. Works with the health care team to provide ongoing nutritional assessment of inpatients including recommendations for diet or alternative nutrition, diet counseling and provides nutrition education materials while documenting progress in the EMR. Provides nutrition education to patients who are referred by their provider for counseling related to specific diagnoses that benefit from modified diets and provides educational materials for home. Nutrition counseling and patients' response will be documented in EMR. This person will also develop and conduct community nutrition education. Responsibilities of this position include:

For Inpatient, evaluates nutritional status of all new patient admissions using pertinent data including diagnosis, history, laboratory values, medications, and anthropometric measures. For Inpatient, documents initial nutrition care plan using standard assessment format after determining patient's nutritional status and needs. Reevaluates patients regularly per policy and changes nutrition plan as needed after reviewing patients medical record and documents in EMR. Promotes seamless care to the patient by effectively communicating the RESPECT values with staff, patient, and providers. Professionally answers telephone, takes messages and direct phone calls based on clinic process in a timely manner. Greets all persons entering the clinic and provides assistance based on their needs. For Inpatient, clarifies diet orders and adds nutrition supplements to best meet patient needs based on medical diagnosis and history. For Inpatient, works collaboratively with TPN team (pharmacist, physician, and dietitian) to determine patient's calorie, protein and fluid needs and documents recommendations in EMR. Makes recommendations and writes orders for alternative nutrition (TPN and enteral feeding). For Inpatient, attends and actively participates in multidisciplinary Daily Patient Care Meeting offering nutrition input to patient care. Rounds regularly to determine patient's intake and encourage healthy diet. For Inpatient, educates patients and family on healthy diet and modified diet based on medical diagnosis and history as well as communicating appropriately based on their level of comprehension. Refers inpatient to outpatient education as appropriate. For Outpatient, evaluates nutritional status of patient using pertinent data including diagnosis, history, laboratory values, medications, and anthropometric measurements. For Outpatient, educates patient and family on healthy diet and modified diet based on medical diagnosis and history. For Outpatient, documents initial nutrition care plan using standard assessment and education format in EMR and communicates with provider regarding progress of the patient. Prepares and presents a minimum of two community nutrition education programs per year including classroom presentations, radio shows and written articles. Ongoing continuing education to remain current in nutrition field and remain registered is mandatory. Requirements for this position include:

Bachelor's Degree. Registered with the Academy of Nutrition and Dietetics and certified with the state of WI. Able to communicate verbally and in writing with all customers and possess the nutrition knowledge to educate all ages for general health and a variety of medical conditions requiring modified diets. Must be organized with high attention to details and team player. Knowledge of computers including Microsoft Office.
