SUMMARY:
Under the direction of the Director of Clinical Services, the Triage RN will provide clear, safe and
effective telephone triage using evidence-based processes and tools. The Triage RN will ensure all calls are addressed in a timely manner, utilizing the highest level of customer service and skill.
Hybrid opportunity may arise for the right candidate showing a proven work ethic.
A high value will be placed on integrity and compassion as most of the incoming calls will be from Hospice and Palliative patients.

Associates in Nursing, Bachelors Degree preferred
Three years in med/surg, palliative, hospice and/or home care; or
A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position
Access to operable, insured motor vehicle and valid driver's license with acceptable record of safe driving practice
ESSENTIAL FUNCTIONS: Screen and triage patient/staff phone calls in a timely and professional manner Collect, analyze and create a well-documented plan of care for the patient based on their presenting symptoms and expected healthcare outcome. Document triage calls in the Electronic Medical Record (EMR) using SBAR format. Communicate with the Case Manager and appropriate team members any necessary information regarding patients' change in status. Assist clinical staff with communication to Providers office, including but not limited to: notification of admission to or discharge from program, obtaining laboratory orders, updating Provider on patients current status or change in level/transition of care. Order medications through Enclara and check status of deliveries for patients Order supplies through Medline and check status of deliveries for patients Assist with ordering of Durable Medical Equipment Assist with chart audits and data collection as needed. Demonstrates good organizational skills and exceptional customer service skills Uses equipment and supplies in an appropriate and cost effective manner Acknowledges that all information related to employees, clients and their families, and NCHHHA business practices is confidential Practices proper body mechanics and observes safety measures Adheres to all safety policies including reporting/ removing hazards immediately Demonstrates knowledge and compliance with infection control policies Follows instructions, responds to management direction Works cooperatively with other NCHHHA personnel Complies with NCHHHA's Standard of Conduct Required completion of time sheets and other records in accordance with agency policy and State and Federal regulations Other duties as assigned LIMITED OR RESTRICTED FUNCTIONS/RESPONSIBILITIES:
The following functions and responsibilities will NOT be provided by the Triage RN:
Acting as a guardian or Power of Attorney for an agency client May not sign checks Contracting in a private duty capacity with an active Agency client. Other functions/responsibilities as may be restricted Transportation of clients.
