For over 25 years, Southwest Care Center has been providing exceptional healthcare to the communities of Santa Fe and Albuquerque, NM. We are a non-profit, FQHC-LAL providing medical, behavioral health, case management, community outreach, diabetes education, and pharmacy services. We are proud to offer gender equity medicine, syringe exchange, and HIV/HepC treatment, testing and other services within our communities.
Southwest Care Center is currently seeking a full-time Community Health Worker-Diabetes. This position supports the Alameda Clinic in Santa Fe; therefore, candidate must reside in the local market area of Albuquerque or Santa Fe, New Mexico.
Position Details:
Under the direction of the Clinic Manager, the Community Health Worker - Diabetes delivers culturally appropriate health education, information, and support to patients and families, with an emphasis on diabetes management and prevention. In addition, they assist patients in accessing necessary care, offer informal counseling and guidance on health behaviors, advocate for individual and community health needs, and provide direct services such as screenings. This role is essential to improving patient outcomes, safety, and experience while ensuring compliance with Southwest Care Center's Standards of Care.
Position responsibilities include but not limited to:
Utilizes effective communication and interpersonal skills to provide information, education, and support to patients and families; performs community outreach, responds to referrals, and addresses requests for diabetes-related guidance and assistance.
Gathers necessary information by interviewing patients, family members, and others, ensuring accurate documentation of diabetes care details and related information.
Enhances program visibility and effectiveness by conducting community outreach, participating in diabetes-related education initiatives, and collaborating with community providers and agencies for referrals and support.
Provides counseling on diabetes management, including nutrition, prevention, and proper use of medications and treatment supplies to help patients achieve optimal health outcomes.
Educates patients on diabetes-related topics, including physiological processes, signs and symptoms of diabetes complications, diet and nutrition options, and the management of daily activities. Refers patients to appropriate community resources for additional support, including financial assistance programs.
Stays updated on regulations, legislation, and enforcement guidelines related to diabetes care and public health.
Reports unusual patient responses, signs, symptoms, or findings promptly to the appropriate licensed healthcare provider.
Participates in staff meetings, diabetes-related training sessions, and attends other meetings or seminars as requested.
Performs other related duties as assigned to support the delivery of quality diabetes care and patient outcomes.

Candidate Highlights:
Six months in a healthcare related field, knowledge of diabetes disease process preferred.
High School Diploma or GED equivalent.
Certified Community Health Worker or successful completion of the Promotores/Community Health Worker Certification required within 12 months of hire.
BLS Certification required.
Bilingual in Spanish Preferred.
