Salary and Benefits The pay range for this position is based on experience ($105,000 - $130,000/year) Health Insurance – Employer pays 75% (Employee: $66.19-$151.86 biweekly) Dental and Vision Insurance Covered 100% 403(b) Retirement Package – with 3% match 12 Days Paid Time Off (PTO) 11 Paid Holidays Continuing Medical Education (CME) – 5 Days Approved with $2,000 allowance biennially 2 Professional Associations – up to $1200 Eligible for Student Loan Repayment -

up to $100,000 Life Insurance Covered - $50,000 Long-term disability Malpractice insurance covered Full support staff Responsibilities Monday – Thursday (8:00 am – 5:30 pm) & Friday (8:00 am – 12:00 pm) Educate patients on preventative care & prescribed treatments Collaborate with the care team in assessing, planning, and implementing the patient’s care plan Must be able to see patients of all ages.
