An Overview
The Crisis Outreach Case Manager conducts outreach for and provides support to individuals in the City of Alexandria that have recently experienced a behavioral health crisis, to ensure stability, connection to services, and engagement in services. Responsibilities include receiving and triaging referrals for outreach, conducting outreach by phone and in person in the community, assessing for immediate needs during outreach, completing referrals, ongoing coordination with referring parties, and documentation. The position collaborates and coordinates with multiple referral sources including, but not limited to, first responders, state and private psychiatric facilities, jail mental health, and DCHS Emergency Services. The Crisis Outreach Case Manager is also responsible for maintaining databases, completing reports, data entry, participation in meetings with team and stakeholders, and may be requested to provide support to DCHS Emergency Services.

The ideal candidate will have extensive experience providing support and case management services to adults and youths experiencing and having experienced behavioral health crisis; strong knowledge of agencies, prevention and diversion resources, behavioral health services, and social services available to City of Alexandria residents; skills in coordinating and collaborating with local and regional stakeholders; ability to work well on a team; excellent verbal and written skills; and experience working with and providing services to culturally diverse populations.

The Center for Adult Services (CAS) provides programs and services in Aging; Adult Protective Services; Clinical & Emergency Services; and Community & Residential Support Services for seniors and for persons with mental illness, intellectual disabilities and substance use disorders. CAS provides compassionate and effective person-centered services based on best practices. Our teams support self-determination, safety and recovery for Alexandria residents affected by abuse, neglect, mental illness, intellectual disabilities, and substance abuse disorders.
Minimum & Additional Requirements
Bachelor's Degree in psychology, social services, public health, guidance, or a related field; three years of experience in the provision of mental health and/or substance abuse treatment services, including some experience at the level of Caseworker; or any equivalent combination of experience and training which provides the required knowledge, skills and abilities. Employees must possess a valid driver's license issued by the State of their residence.
Preferred Qualifications
Registration as a Qualified Mental Health Professional - Adult (QMHP-A); at least two years of experience in providing mental health case management to adult and youth populations; at least one year of experience in crisis intervention; or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities. Bilingual (English/Spanish) to meet needs of population served. Proficiency with Microsoft Office, Excel, Publisher, Word, and Outlook.

The Opportunity
- Conduct outreach for and provide short-term support to individuals in the City of Alexandria that have recently experienced a behavioral health crisis;
- Determine post-crisis needs for referred individuals and provide appropriate resources and referrals;
- Receive and triage referrals for post-crisis outreach from multiple sources, including but not limited to first responders, state and private psychiatric facilities, jail mental health, and DCHS Emergency Services;
- Coordinate and collaborate with internal and external referral sources;
- Complete necessary documentation, data entry, and tracking;
- Maintain clinical records in accordance with State, City, Departmental and Federal compliance requirements;
- Participate in meetings with team and stakeholders;
- Handle time-sensitive and confidential information;
- As needed, provide support to DCHS Emergency Services team;
- Perform related work as required
