Position Summary
Under the supervision of the Department of Social Work, provides direct supportive services and connections to recovery resources to adults in recovery from a substance use disorder. Serves as a role model, mentor, advocate, and motivator to individuals in order to promote long-term recovery. Works as a member of the care team by engaging patients in recovery planning, including identifying treatment services and facilitating referrals. Maintains contact by phone and/or email as well as in-person contact with recovering person after they leave the hospital to ensure their ongoing success. Provides re-engagement support with other agencies.
Principal Duties and Responsibilities
- Performs and documents delegated patient care activities
- Performs patient care activities as defined in the skill competency and scope of practice
- Safely and independently completes patient assignment
- Identifies patients who need consult based on electronic health record
Requirements
- High School Diploma or GED. PRC-focused education and training preferred.
- Must have lived experience of recovery from substance use.
- Must have skills to establish supportive relationships with persons experiencing substance use and co-occurring mental health disorders.
- Valid driver's license preferred
