Duties

A community health worker (CHW) is a trusted member of the community they serve and has a close understanding of its ethnicity, language, socio-economic status, and life experiences. CHWs are frontline public health workers who act as a liaison between the community and health and social services. Their trusting relationships with the community allow them to help people access services, improve the quality of service delivery, and address barriers that underserved communities face. A community health worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
Qualifications

  • Community Health Worker (CHW) Certificate through the Ohio Department of Nursing is required or must be willing to become certified within 6 months of employment by FCHD.
  • Strong interpersonal and communication skills.
  • Knowledge of community resources and services.
  • Ability to build and maintain trusting relationships with community members.
  • Cultural competency and sensitivity, empathy, compassion, and dedication to serving others.
  • Commitment to maintaining client confidentiality and ethical standards.
  • High school diploma or GED is required. Bachelor's degree in public health, public administration, social work or other human services related field preferred.
  • Training in Emergency Preparedness to be completed upon hiring.
