Overview

We at MHealth Fairview are looking for you to join our University of Minnesota Medical Center located in Minneapolis ! This benefits eligible position is a 1.0FTE (80 hours/2 weeks) on the day shift (8am-4:30pm Mon-Friday, no weekends) .
The Community Health Worker (CHW) is responsible for helping community members, patients and their family’s access and navigate community services and resources and adopt healthy behaviors. The CHW supports providers and team members through an integrated approach to care management and community outreach. Activity will promote, maintain, and improve individual and community health. The CHW provides social support and informal counseling, individual and group health education and advocacy for individuals and community health needs. Providing home visits and health screenings may be required.
The Community Health Worker works with the unit providers, nurses and care management team on all aspects of discharging patients from the hospital.
Responsibilities Job Description

Promote wellness by providing culturally appropriate health information to providers, patients and/or community members

Refer and link to preventive services through health screenings and healthcare information

Assist with connecting patients to community resources upon discharge from the Emergency Department

Schedule Primary Care Provider Appointments for patients after discharge from the Emergency Department

Conduct follow-up calls for patients 48-72 hours after discharge from the ED, 1 week after their follow-up PCP appointment and again 30 days after discharge from the ED

Serve as liaison between providers, nurses and the care management team

Address social determinants of health
Qualifications
Required
Experience
3 years’ experience working on community health projects, programs, or initiatives
License/Certification/Registration
Community Health Worker Certificate will be required within 18 months of hire
Preferred
Education
Current Community Health Worker Certificate
College degree in health, human services, or related field
Experience
Teaching or presenting information to groups
Experience with Microsoft Office
Multicultural healthcare experience or healthcare experience in other countries
