Working Conditions:
Work is primarily in an office environment and occasionally outdoors in varied weather conditions. Work may involve exposure to excessive noise and hostile individuals. While performing the duties of this job, the employee regularly is required to stand, walk, and sit; use hands to finger, handle or feel; reach with hands or arms; and talk or hear. Ability to lift up to 20 lbs. Clarity of vision at short distance is required. Evening, holiday, and weekend work may be required. Subject to random pat down searches.
Conditions of Employment:
Selected candidate must submit to and pass a County paid pre-employment physical and drug/alcohol screening. Additionally, selected candidate must submit to and pass a county paid criminal background screening (driver's license, criminal history, and wanted persons). Selected candidate must possess and maintain a valid New Mexico Class D Driver's License as incumbent shall be appointed to drive a County vehicle during the performance of his/her duties. Candidate must obtain and/or maintain all certifications required.

Primary Purpose
To coordinate the successful community re-entry and appropriate clinical services for clients discharged from the Santa Fe County Adult Detention Facility.
Essential Job Functions
- Oversees various functions regarding the health and aftercare needs of detainees leaving the Santa Fe County Adult Detention Facility which assists the detainee in transitioning into the community. The position will work with Leadership and will report directly to the Programs Manager.
- Coordinates efforts to ensure detainees transitioning back into the community have been enrolled in Medicaid or comparable health insurance programs through the Health Insurance Exchange prior to release from incarceration.
- Develops a standard protocol for enrollment in Medicaid or other health insurance to support successful re-entry into the community.
- Collects, enters, and analyzes data on the number of discharged clients enrolled in Medicaid or other comparable health insurance program.
- Works cooperatively with other Santa Fe County agencies who are working to ensure residents have access to health insurance.
- Develops and implements a system of discharge planning that provides a mechanism for discharged inmates to have knowledge and access to pertinent community-based services, including housing, medical care, and other necessary support services.
- Coordinates the development and implementation of an opioid overdose prevention education service model to include the dispensing of Narcan rescue kits upon release, targeting detainees at risk of opioid overdose
- Establishes means by which opioid dependent detainees are identified (completion of 5-day or 10-day opiate withdrawal protocols) and targets for service delivery during incarceration.
- Establishes processes whereby family member/rescue person identified by detainees enrolled in program are provided overdose prevention education to include Narcan administration training off site.
- Develops and facilitates training of identified facility staff that will be charged with delivering overdose prevention education and Narcan training to detainees who are transitioning back into the community; detainees and identified family members/care givers.
- Distributes Naloxone kits to those detainees that are at high risk for opioid overdose.
- Collects and analyzes overdose prevention services delivery data to include enrollment, educational services delivery, and dispensing of Narcan rescue kit.
- Participates in leadership meetings by sharing observations and recommendations regarding the facility, participants and staff in order to identify and solve problems and function as a unified team.
- Maintains confidentiality regarding employees and participants.
- Develops monthly reports with data on health insurance enrollment and overdose prevention service delivery.
- Knowledge of common health-related needs including substance use/opiate addiction, overdose prevention, mental health, Medicaid and other health insurance programs.
- Knowledge of the Medicaid system in New Mexico and New Mexico's efforts to increase enrollment in Medicaid.
- Skill in communicating with clients in an open, non-judgmental manner using non-technical language.
- Skill in making effective decisions in emergency situations.
- Skill in developing and maintaining plans and schedules.
- Skill in training personnel.
- Skill in organization and prioritizing tasks to keep work flowing in an orderly and timely manner.
- Ability to establish and maintain effective working relationships with associate personnel and community partners.
- Ability to communicate effectively, both verbally and in writing
- Ability to operate various word-processing, spreadsheet, presentation, and database software programs.
- Ability to prepare accurate, complete and legible reports in English using correct grammar; to add, subtract, multiply, divide, and calculate percentages.
- Bachelor's degree in Public Health, Social Work, or related field and a minimum of three (3) years of experience performing duties relevant to the essential functions and primary purpose identified herein. Related education and experience may be substituted at a rate of thirty (30) semester hours equal to one (1) year experience.
- PEEMOSA training preferred.
