Job Responsibilities
With our Agency's mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina.
This position is located at the Department of Mental Health, Lexington County Community Mental Health Center, 301 Palmetto Park Blvd, Lexington, SC 29072. Services are provided in the community and in patient's homes in Lexington County.
Responsibilities for MHP I - Assertive Community Treatment - include but are not limited to:
Under general supervision, serves on the Assertive Community Treatment (ACT) team providing evidence-based services (safe, effective, and measurable) to promote and support the recovery of persons diagnosed with a serious mental health condition. Guides patients in development of skills or strategies toward achieving and maintaining good mental health.
Job Functions
Minimum and Additional Requirements
A Master's degree in social work, counseling, or related field.

Preferred Qualifications
Ideal candidate will be licensed by the State of South Carolina as a LPC, LPC-A, LMSW, LISW-CP, or LMFT and have at least 1 year of post-graduate experience.
Bilingual abilities in English and Spanish (or another language) are a plus.

Must be able to lift 20 lbs.

Job Functions
- Provides outpatient counseling and treatment services to patients based on their individual goals and level of care. Mental Health services will include, but not be limited to: assessment, crisis management, individual therapy, group therapy, and family therapy.
- Works with ACT team members to meet fidelity requirements and ensure all patients receive appropriate number of contacts each month.
- Ensure patient functionality is measured by the DLA-20 (Training and certification is provided upon employment).
- Completes clinical/administrative documentation according to Quality Assurance/SCDMH policies to include setting patient goals, developing treatment plans, measuring and documenting progress, and completing prior authorizations.
- Participates in clinical supervision, treatment team meetings, pursues professional development, and demonstrates effective use of social and interpersonal knowledge and skills.
- Reviews caseload weekly for timely discharge, engagement, transition, and follow-up
- Complies with SCDMH Policies and Procedures to include but not limited to Corporate Compliance and Risk Management.
- Pursues opportunities to further skills and knowledge as a therapist. All services are provided with a focus on diversity and inclusion.
- Performs other duties as needed.
