Job Description

This position has overall responsibility of facilitating organizational improvement and support safety across NYCHealth+ Hospitals/Jacobi. Under the direction of the Patient Safety Officer, supports and promotes patient safety initiatives and efforts by conducting data analyses; reviews and coordinates patient safety programs. Works with leadership and health care staff to analyze and improve patient safety practices. Acts in accordance with NYC Health +Hospitals, applicable local, state, and federal rules, regulations and patient safety standards. Contributes to continuous improvements to patient safety and compliance activities. There are two (2) Assignment Levels within this class of positions; all personnel perform related work. The Patient Safety Spec Level II uses safety driven principles to support a just and reliable culture, and is responsible for monitoring project goals and milestones, achieving sustainable positive outcomes from improvement work, and for identifying opportunities to spread standardized best practices. The Patient Safety Specialist Level II will work closely with leaders across Jacobi to understand strategic direction and priorities to promote Patient Safety.

Essential Duties and Responsibilities:

  • An important aspect of the patient safety associate position is the ability to work collegially and collaboratively with a large number of groups and individuals. While the patient safety associate has the primary responsibility for the items listed below, they should be accomplished by working closely with the groups and individuals listed above.
  • Works with the Patient safety Officer to provide leadership and direction to the organizational patient safety strategy.
  • Continuously evaluates and improves the patient safety goals based on internal needs and external requirements and standards.
  • Responsible for completing multiple quality and safety related surveys and reports
  • Coordinates and manages patient safety data collection and submission, analyzes and summarizes data, prepares statistical and/or other appropriate reports; ensures completeness and timeliness of patient safety reported data; informs appropriate leadership of data findings, including Leapfrog Survey and Culture of Safety Survey.
  • Investigates and provides summaries of "Good Catches"/near misses. Prepares action plans for improvements surrounding "Good Catches"/near misses' findings.
  • Observes for hazard risk reduction opportunities through active rounding and other strategies.
  • Implements safety programs for harm reduction in the facility
  • With appropriate collaboration, assists in the development of a comprehensive Patient Safety Plan and revises the plan as needed.
  • Conducts an annual assessment of patient risks and through working closely with the Patient Safety Council and Senior leadership, develops annual institutional patient safety goals.
  • Ensures the development of surveillance systems to identify and mitigate potential patient harm.
  • Plans, coordinates and monitors comprehensive safety programs/ dashboard and projects.
  • Supports/facilitates the implementation of innovation and systems that eliminate patient harm.
  • Monitors literature and professional organizations for new or revised best practices related to patient safety.
  • Demonstrates knowledge of evolving and best practices related to the applicable clinical area and related performance improvement and patient safety. Works to expand knowledge of quality and safety bringing new ideas, new concepts, and innovative models of healthcare delivery to the healthcare system
  • Facilitates process improvement teams that directly support NYCH+H patient safety goals
  • Develops and implements initiatives to create an appropriate patient safety culture using transparency as the foundation of this effort
  • Supports and encourages submission of patient safety learning reports from frontline staff.
  • Reviews event reporting system and analyzes entries to address patient safety concerns and identify trends and actively collaborates with involved teams and/or departments.
  • Works in collaboration with Patient Safety Officer, Risk Management and senior hospital leadership to coordinate root cause analysis of patient safety events and assures implementation of resulting actions plans.
  • Participates in state and federal regulatory surveys and survey preparation process.
  • Coordinates organizational response to regulatory standards related to patient safety including but not limited to the TJC National Patient Safety Goals.
  • In collaboration with Patient Safety Officer and Human Resources, designs, updates, and implements patient safety education modules for new employee orientation and continuing education
  • Assists in the responsibility for communication and education regarding safety and quality activities in the patient care departments, as well as the medical community, governing body, community, and serves as a resource.
  • Models a patient-centered, interdisciplinary team approach to improvement and safety that values transparency.
  • All projects focus on optimizing patient care from safety, quality, and patient experience perspective.
  • Supports a Just Culture environment and non-punitive error reporting
Minimum Qualifications

For Assignment to Level II:
1. Valid New York State license and current registration to practice as a Registered Professional Nurse.
2. Four (4) years clinical experience in coordinating and monitoring initiatives, activities, and programs related to patient safety and/or quality improvement or performing other functions directly related to patient safety in a hospital or health care setting

Department Preferences

Master's Degree preferred

2 years of clinical experience in coordinating. Initiates activities related to patient safety.

Rotating Tour: Monday-Friday
