Acenda Integrated Health is recruiting for a Counselor to support the Functional Family Therapy Program in Cape May, New Jersey.

Job Overview: The case manager engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community. This is done through direct in-home contact and linkage to community resources. The consumer is linked to resources that support the ability to remain stable within the community.

Key Responsibilities:
- Provides emotional support, appropriate intervention strategies and adjust services accordingly.
- Builds positive interactions, enhance child, parent and family well-being and stabilize the home and family.
- Engages each family with specific goals designed to develop and strengthen the unique characteristics of that family; taking a relational approach.
- Addresses safety concerns and enhance functioning and well-being of children and families residing in the following counties: Atlantic, Cape May, Cumberland, and Salem.
- Bachelor's or advanced degree in counseling, social work, psychology, or related field from an accredited college or university. Prior in-home experience preferred.
- Must have experience with crisis intervention and conflict resolution.
- Must be able to exercise independent thinking and good judgment under all circumstances.
- Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
- Must have and maintain a valid driver's license with an acceptable driving record.
