• WA state license; active, unrestricted

• Board Certified Obstetrics & Gynecology; Board Eligible (2-5 years post training).

• DEA

NC/PRES with:

• Delivery volume over the last 2 years.

• NPDB self-inquiry report: The NPDB - Home Page (hrsa.gov) + explanations (if applicable).

• Updated CC (with MM/YY; all gaps over 30-days explained)

• 14 shifts per month of availability (weekdays + weekends).

COMPENSATION:
