Purpose

To provide housing case management to SIF assisted clients seeking housing and services in the New Haven region. This position will primarily support clients in the housing application process, housing search, and independent living with the ultimate goal of self-sufficiency and housing stabilization.

Duties and Responsibilities

Assist and monitor SIF clients from entry into the program until successfully discharged in collaboration with other community providers. Outreach and engage clients as necessary; provide case management services. Apply for state housing assistance (SIF/ RAP) and assist with securing affordable housing, ongoing check-in with clients to meet their housing needs. Assist with maintaining or creating connections with mental health, substance abuse and other health care providers. Maintain client records in a manner consistent with projects and agency standards including both paper and electronic records. Interact with landlords on behalf of clients as needed. Manage a client case load of approximately 20 individuals at any point in time. Other related duties as assigned. Qualifications, Knowledge, Skills and Abilities

Individual must be able to work both independently and within a team setting. Experience in case management, supportive housing or a related field required. Knowledge of Fair Housing Laws; sound judgment and decision making; and Solid math skills required. A strong candidate is highly organized and attentive to detail and is motivated to complete tasks and see consumers succeed. Bachelor's degree required, excellent writing skills, must have a valid Driver's License with an acceptable driving record; computer literate; must be able and willing to accept direction and guidance from supervisors. Salary Range:

$25.50 to $27.50 per hour plus excellent benefits

Schedule:

Monday - Thursday 8:00 a.m. to 4:00 p.m.
