Description
We are looking for a Clinician Facilitator for our Flexible Integrated Treatment (FIT) program in Elk Grove.

Program Summary:
FIT serves infants, children and youth (ages 0-21) with a team approach to treatment planning allowing clinicians the flexibility to assess the level and type of services needed. As treatment needs vary over time, services are tailored to fit without interruption. Equally flexible is the location of service: home, school, River Oak clinical sites or community setting to best accommodate the family/care provider. A wide array of treatment interventions, to include multiple evidence-based programs, are used to include individual, family and group therapy.
Job Responsibilities:
Provide strength-based and individualized coordination of care with youth, families, internal team members and community partners to affect positive changes Lead Child and Family Team Meetings Participate in on-going consultation, coordination of care, required documentation, and systematic support with treatment team in the implementation of Plan of Care for individual youth and families Perform home, school, office and community visits to observe and assess the youth in the appropriate setting(s); plan and participate in activities; listen to the youth and/or family in a supportive and outcome-based manner; empower the youth and family; perform crisis management and on-call duties Maintain HIPAA compliant electronic systems and records Meet established billing goals; track productivity levels on a consistent basis Maintain awareness of and participation in ongoing professional development Practice cultural consideration, concern and courtesy to youth, families, community partners, and other staff members when giving appropriate information, advice, and assistance Maintain professional relationships with all team members Minimum Qualifications:
Must have at minimum:
Bachelor's degree from an accredited university or college with major course work in social work, psychology, counseling, or closely related field.
PLUS, at least four (4) years full-time experience in providing mental health services/social services case management or mental health counseling; services must be provided in mental health, community-based, or probation setting;
OR
Possess a Masters Degree from an accredited university or college with major course work in social work, psychology, and counseling or a closely related field
PLUS, at least two (2) years full-time experience in providing mental health services/social services case management or mental health counseling; services must be provided in mental health, community-based, or probation setting
Please note: **Experience in an academic counseling setting does not qualify** Must have excellent organizational, analytical, communications, and care coordination skills Must be able to empower people with a strength-based approach, prioritize responsibilities, and build rapport with a wide variety of individuals Ability to meet the professional standards and requirements for assigned duties and responsibilities Proficient with Outlook, Microsoft Word, Excel and PowerPoint Maintain a variety of online recordkeeping and documentation processes and systems for assigned clinical program; ensure all relevant confidentiality and privacy requirements are met Benefits:
