Details Client NameSanford Health Equip Job TypeTravel OfferingAllied ProfessionTherapy SpecialtyPT Job ID29966234 Job TitleHome Health Physical Therapist Weekly Pay$1986.0 Shift Details ShiftDay - 8x5 - 08AM Scheduled Hours40 Job Order Details Start Date01/20/2025 End Date04/21/2025 Duration13 Week(s) Job Description Job Title: Home Health Physical Therapist Job Specialty: Therapy Job Duration: 13 weeks Shift: 8:00 am - 4:30 pm Guaranteed Hours: 40 hours per week Experience: New graduates welcome License: National registration and licensure required for the state of practice Certifications: Completion of an accredited physical therapy program required Must-Have: Basic physical therapy skills, ability to communicate effectively in English Job Description: - Assess clients' needs and develop therapy plans to meet identified requirements.- Administer and perform therapeutic interventions with clients.- Maintain clients' self-esteem, privacy, and confidentiality of personal information.- Provide clients with opportunities for independence and choice consistent with their care plans.- Notify physician of changes; process orders accordingly.- Document observations and services provided accurately.- Provide direct consultation with physicians and/or interdisciplinary team members; share information appropriately.- Provide training on programs, adaptive equipment, and other interventions.- Utilize and supervise therapy assistants in accordance with Federal and State regulations; document appropriately in clinical records.- Follow infection control and safety standards when using equipment; utilize appropriate bag techniques and standard precautions.- Comply with all related laws, regulations, company policies, and procedures.- May supervise one Physical Therapist Assistant (PTA).- Cases per day: up to 5 in single-family homes, apartments, or assisted living facilities within a 30 to 40-mile radius.- Use Remote Client/Epic software.- Productivity expectations: 85-90% once trained.- Travel mileage and drive time reimbursed. Work Environment: - Moderate noise level- Physical requirement: Ability to lift 50 lbs- Reasonable accommodations may be made for individuals with disabilities to perform essential functions.
