Job specification
Lead and manage the Case Management department, including overseeing staff recruitment, training, scheduling, and performance evaluation. Oversee case management activities to ensure effective discharge planning, utilization review, and care coordination across the continuum of care. Collaborate with interdisciplinary teams to develop comprehensive care plans that meet patient needs while ensuring cost-effective care. Provide leadership during case conferences, fostering collaboration and proactive problem-solving to address complex cases. Manage day-to-day operations of the Case Management department, ensuring resources are utilized efficiently and processes run smoothly. Candidate requirements
A Bachelor of Science in Nursing (BSN) is required; a Master's degree in Nursing (MSN) or Healthcare Administration is preferred. Current Registered Nurse (RN) license in the state of Texas. Minimum of 5 years of clinical nursing experience, with at least 3 years in case management or care coordination. Previous leadership or supervisory experience in case management is required. Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification is preferred.
