STD/HIV Community Health Worker $40,594 - $45,659/ Year Full Benefits Package Duties: Identifies and locates individuals with STD/HIV diagnoses in order to support their needs in obtaining or sustaining health care treatment to achieve positive health outcomes. Assists clients with navigating the health care system and accessing available resources such as the Ryan White Care Act Program. Plans, coordinates and implements specific interventions aimed at re-engaging STD/HIV-positive clients who are lost to care. Establishes trusting relationships with community members, clients and families from diverse populations while providing support and encouragement. Acts as a client and community advocate. Maintains a presence in the community through outreach and health education, community events, and strong professional partnerships with area organizations serving overlapping clients. Provides education involving health issues such as HIV/AIDS, substance abuse, chronic disease, behavioral health, child rearing, parenting or other health concerns. Provides program outreach such as home visiting activities, individual and community assessments, and health screening events. May deliver presentations at local agencies and community events. The following training will be provided upon hire: HIV Test Counselor Certification Training (Modules I, II, III), STD Treatment Guidelines and Early Intervention Services for People Living with HIV. It may not be a comprehensive list of all duties and responsibilities required by a person so classified. Education and Experience: High school diploma with college experience preferred. Experience in a health-related field, preferably in public health dealing with high-risk individuals. Desired: Certification as a Community Health Worker (CHW). HIV Antibody Pre/Post Test Counselor certification. Phlebotomy training or the willingness to learn. Special Requirements: Must possess and maintain a valid driver's license throughout the course of employment. Non-traditional work hours may be required on occasion. In times of a public health emergency, may be required to report for specialized assigned duties inside or outside of Berrien County. Required Knowledge and Skills: Computer proficiency in Windows-based programs and Microsoft Office Suite. Knowledge of HIV/STDs and the areas and activities where high-risk individuals may be found to provide outreach activities. Aware of HIV/STD community resources, including existing social and behavioral health resources and referral agencies. Must be comfortable working with high-risk groups, including infected individuals and affected populations. Sensitivity and awareness of diverse socioeconomic, cultural, ethnic and health issues. Capable of maintaining client confidentially and client rights according to the Health Insurance Portability and Accountability Act (HIPAA). Excellent communication skills to deliver information and gain cooperation/trust from clients and community members. Strong time management skills, with the ability to handle multiple responsibilities efficiently.
