POSITION SUMMARY: Responsible for coordinating the provision of services on behalf of persons with Intellectual/ Developmental Disabilities, in gaining access to needed services, i.e., social, medical, educational, vocational.

QUALIFICATIONS: Bachelor Degree in psychology, social work, or related field in human services; preference for work experience with IDD population

SKILL SETS: Possesses superior organizational, oral, and written communication skills. Must demonstrate a high level of familiarity with Microsoft Office and exceptional customer service skills. Must be dependable, interact effectively with the public and staff, adaptability-able to deal with frequent change, delays or unexpected events, strong sense of commitment.

PHYSICAL AND ENVIORMENTAL DEMANDS: Frequent talking and active listening when communicating to individuals, families, guardians, and staff. Possible exposure to communicable diseases, possible exposure to blood borne pathogens. Visual acuity necessary for appraising the appearance of individuals and their environment. Conduct home visits with individuals which may cause exposure to risk of unsanitary conditions, abuse and violence. Work is light, standing and sitting, occasional stooping. Ability to handle stress effectively and make decisions accordingly.

JOB SPECIFIC FUNCTIONS:

  1. Thorough documentation on monthly face to face visits, monitoring of identified services and any crisis services. Serves as an advocate for the individuals in conformity with PL.95-602, Section III. Responds by phone in a timely manner to crisis calls and advises caller of possible options.
  2. Facilitates, reviews and updates Support Plans and Person Centered Plans. Participates as an active member of the Support team which develops the support plan and/or facilitates for Person Centered Plans for those individuals who receive funding through the SGF, SCL, Crisis Response, Individualized Supports or PASRR Specialized Services Programs. Teaches, trains, and monitors the development of service plans and Person Centered Plans through the Support team. Plan, identify and implement support strategies for individuals which includes principles of empowerment, community inclusion, health and safety assurances, and use of informal supports. Assists in development of client service plans. Coordinates and arranges for needed services as identified in treatment plan.
  3. Assists families and/or individuals in crisis situations which may result in admission into a facility to meet their needs.
  4. Promotes the policy of human rights and attends Human Rights Committee Meetings. Participates in agency continuing quality improvement (CQI) activities.
  5. Performs other duties as assigned.
MANDATORY REQUIREMENTS:

  2. Attends mandatory in-services and other training/educational programs as required.
