Compensation

Bachelor's Degree: $21.30 - $23.38 per hour

Master's Degree: $23.10 - $25.18 per hour

Offers based on education and years of relevant experience

Primary Responsibilities

Maintain a case load of 12 families, providing direct face to face contact with youth and families in accordance with the Family Voices Network and Erie County Preventive Services.

Coordinate services for families enrolled in the Erie County Wraparound Program which includes oversee the aspects of Care Coordination for families enrolled in Wraparound Services

Facilitate all Child and Family Team Meetings (CFT) and ensuring all relevant participants are included

Communicate with all necessary providers and supports related to the planning and care for youth/families

Fulfill other duties as assigned

Required Qualifications

Bachelor’s Degree in Human Services related field with 4 years’ experience providing supportive services to children and their families

Current NYS Driver’s License and personal vehicle

Ability to build rapport with youth based on healthy boundaries, effective communication and mutual respect

Ability to exercise discretion and judgement in decision making

Ability to demonstrate sensitivity, compassion, patience, and active listening

Ability to effectively work with individuals from a diverse array of cultural backgrounds, beliefs, customs, and lifestyles

Preferred Qualifications

Master’s degree in Human Services related field with 2 years’ experience providing supportive services to children and their families
