The role plans, organizes, analyzes, implements and evaluates patient care in the program. The Nurse Coordinator collaborates with the Program Medical Director, multidisciplinary care staff, and other administrators in the development, implementation, and evaluation of all program components. Responsibilities Develops, implements, and enhance special projects promoting inpatient/outpatient continuity of care and parent engagement Ensures the provision of high quality, individualized, comprehensive, cost effective patient care across the continuum Initiates, tracks, analyzes and managers all program quality improvement and research projects, program goals and objectives Responsible for coordination functions with interdisciplinary medical care, liasison functions with patient, family, medical staff, and referring phsyicians Develops educational materials and programs for patients, nursing and ancillary staff, and physicians Develops clinical protocols, policies, and procedures; community and public relations Qualifications KNOWLEDGE, SKILLS AND ABILITIES REQUIRED LICENSURE: Registered nurse in the State of Ohio required EDUCATION: Bachelor's degree required. Master's Degree in Nursing preferred. EXPERIENCE: Three-five years registered nurse experience required. SKILLS: Effective customer service, communication (oral, written, presentation), conflict resolution, problem-solving and interpersonal skills required for interaction with contacts. CERTIFICATIONS: Maintains national certification as APRN in the appropriate population foci for the position, or PA MINIMUM PHYSICAL REQUIREMENTSNote frequency as Constantly (67-100%), Frequently (34-66%) and Occasionally (0-33%) for each requirement.
