Position Summary
The Interfaith Works Drop-in Center (IWDC) and IW Shelter (IWS), located at Progress Place in downtown Silver Spring, is a fast-paced drop-in program with both year-round and seasonal overflow shelter program for adults experiencing homelessness. The program serves as an entry point and provides basic needs (toiletries, showers, laundry detergent and facilities) and case management services. The IWDC also operates an Overflow Shelter Program during winter and hyperthermia months. The
Assertive Case Manager's primary function is to be present during times clients are seeking other services (including meals and shelter) in order to develop rapport and identify clients who are either unable or reluctant to engage in traditional case management and could benefit with further engagement in services. In addition, this role includes the traditional case management responsibilities to meet with clients to conduct prescreens and needs assessments and determine eligibility and connect clients to appropriate services and housing.
Responsibilities:
Builds rapport with the Progress Place community by being present and engaging clients coming for meals, Overflow Shelter and/or basic needs services. Identify clients who we should increase efforts to engage in services. Conducts prescreens, psychosocial and needs assessments to determine client needs and appropriate resources in the community. Provides individual case management for people experiencing homelessness. Collaborates with the Peer Support Staff and Behavioral Health Technician to engage clients in the Progress Place parking lot and provide wraparound case management services to clients not otherwise working with the Progress Place case management team. Partners with external providers and stakeholders including the Central Business District Police, Silver Spring Urban District team members, local behavioral health and medical providers. Works with clients to devise an Individualized Service Plan that outlines the client's goals that can include obtaining benefits, behavioral health or medical treatment and/or housing. Uses clinical skills and knowledge of behavioral health issues to mediate client concerns and de-escalate situations when necessary. Makes appropriate referrals to programs and providers that address behavioral health, medical, legal and housing concerns. Maintains data in the Homeless Management Information System (HMIS). Tracks data as outlined by the Program Director to maintain program statistics. Participates in appropriate meetings within Montgomery County's Homeless Continuum of Care (including bi-weekly Coordinated Entry System calls and appropriate Staff Meetings). Participates in weekly clinical rounds held with the IWDC clinical team. Collaborates with other service providers to achieve optimal outcomes for all clients. Participates in regular clinical trainings. Meets with the Program Director for weekly supervision. Has a flexible schedule that may include hours during breakfast (7:30-8:55am) or dinner (5:30-6:55pm) and later evening hours in the winter when the IWDC Overflow Shelter is open. Other duties as assigned.
Qualifications:
Bachelor's degree in a behavioral health field, relevant job experience or lived expertise.
Knowledge of behavioral health symptoms, family dynamics, human behavior and issues related to homelessness.
Flexibility and openness to working in a fast-paced program serving clients from diverse racial, cultural and socio-economic backgrounds.
Excellent communication skills.
Experience wor ing with individuals with acute and chronic behavioral health issues and knowledge of crisis intervention techniques.
Knowledge of (and/or willingness to learn and explore) community resources related to homelessness.
