Help Them Home Intensive Case Manager

Hours: Monday - Thursday

Status: Full-Time

Reports to: Adult Housing Programs Supervisor

Purpose:

Under the direct supervision of the AHP Supervisor, this position provides direct intensive case management services to chronically homeless adults, families, and youth.

Duties, Functions, and Responsibilities:

  1. Conduct initial interview/intake, and client needs assessments.
  2. Provide case management and supportive services to clients moving into long-term housing, to promote transition from homelessness to residential stability.
  3. Create individualized case plans with each adult member of the household, based on the client's needs, to achieve self-sufficiency.
  4. Review the client's treatment plan monthly, update the client's case plan, and track outcomes.
  5. Connect clients with wrap-around services through other programs offered at HELP, including Workforce Services Job Readiness Workshops, Energy Assistance Program, holiday programs, etc.
  6. Connect clients with mainstream programs, including but not limited to, Medicare, and SSI/SSDI.
  7. Provide clients with referrals and resources for other social services and partner agencies.
  8. Must be able to communicate effectively with individuals from a variety of social, cultural, and economic backgrounds.
  9. Meet with clients monthly and complete income calculations and rent requests before the 1st of the following month.
  10. Conduct weekly/semi-monthly home visits.
  11. Provide transportation to clients to ensure that all appointments are kept.
  12. Enter case notes in true time data (daily) into HMIS, reflecting activities of the client's progress.
  13. Always maintain audit-ready files. (Random audit checks are regularly performed)
  14. Maintain clear boundaries with all clients and providers.
  15. Must never allow your or the organization's ethical standards to be compromised.
  16. Facilitate Life Skills Training sessions for clients on-site 2-3 times per quarter.
  17. Attend meetings and trainings, as required.
  18. Other duties, as may be assigned.

Knowledge, Skills, and Abilities:

  1. Knowledge of local social service agencies and programs, as they pertain to the local homeless population.
  2. Knowledge of applicable statutes, ordinances, and codes.
  3. Knowledge of social service work, and interviewing/referral techniques.
  4. Experience and understanding of homeless issues.
  5. Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.
  6. Communicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding of and sensitivity to culturally diverse populations.
  7. Effective written, verbal, and listening skills. Independently initiates and responds to correspondence.
  8. Ability to maintain order and exercise appropriate judgment in crisis situations.
  9. Is flexible and can quickly adjust to shifting priorities.
  10. Establish and maintain cooperative working relationships with clients, fellow employees, other agencies, and the general public.
  11. Organize work schedule and budget time.
  12. Understand and follow all agency policies and procedures.
  13. General computer knowledge, and the ability to use Microsoft Office software.
  14. Ability to work flexible hours, as required by the client caseload.
  15. Ability to perform accurate mathematical equations, calculate client's rent and utility costs and request funding to pay those bills.

Qualifications, Education, and/or Equivalent Experience:

  1. Bachelor's Degree in Social Work, or High School Diploma/GED with (2) two years of experience, substantially equivalent to a Bachelor's Degree, or combination of education and experience.
  2. Experience working with the homeless population.
  3. Must have a current NV driver's license and a clean DMV record. Drivers' History Reports are required to be submitted on a quarterly basis.

Working Conditions:

Work is performed in an office setting, and at various community and residential locations. May be subject to assisting clients with shopping, laundry, and moving into an apartment that may include, bending, squatting, lifting, pushing, and pulling, and simple grasping.
