Hospice Registered Nurse (RN)

provides compassionate, patient-centered care to patients with terminal illnesses and their families, focusing on improving quality of life, managing symptoms, and providing emotional and spiritual support. The Hospice RN works in a multidisciplinary team to ensure comfort, dignity, and support during the end-of-life journey.

Key Responsibilities :

Provide skilled nursing care to patients with terminal illnesses, focusing on pain management, symptom control, and emotional support. Develop individualized care plans to address physical, emotional, and psychosocial needs, ensuring patient comfort and dignity. Administer medications, including pain relief, symptom management, and hospice-related treatments. Educate patients and families about hospice care, prognosis, treatment options, and emotional support. Collaborate with physicians, social workers, chaplains, counselors, and other healthcare professionals to provide holistic care. Monitor and assess patients for changes in health status, adjusting care plans as necessary. Provide emotional, spiritual, and psychological support to patients and their families, including end-of-life counseling. Offer bereavement support to families after the patient's death. Ensure patients and families understand their options for care, including advance directives, palliative care, and end-of-life planning. Document patient care accurately, including symptoms, treatments, and progress, in compliance with hospice care standards. Provide education and support to caregivers and family members on caring for the patient in the home setting. Promote infection control, safety, and dignity in end-of-life care settings. Qualifications :

Education : Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted. Licensure : Valid Registered Nurse (RN) license in the state of practice.

Certification : Basic Life Support (BLS) certification required. Hospice and Palliative Care Certification (CHPN) preferred.

Experience : Previous experience in hospice care, palliative care, or related healthcare settings
