Social Worker - Trilingual English, Cantonese, and Mandarin

Schedule: Full Time; M-F 8am-5pm

Pay: $45.17 - $50.18 per hours.

Job Type: Temp to Hire

POSITION SUMMARY:

The Social Worker provides direct case management services to Client's participants. The primary role of the Social Worker is to assist participants and their family/caregivers to cope with the social and emotional issues and practical arrangements related to maintaining their independence.

DUTIES / RESPONSIBILITIES:

  • Utilize independent judgement and discretion to provide case management to a panel of participants.
  • Conduct initial and routine psychosocial assessments on new and existing participants in order to develop, implement and update individualized care plans and interventions.
  • Partner with caregivers and support system to develop care plan and deliver services.
  • Identify risks and other psychosocial problems.
  • Identify service needs of each participant; advocate, refer, coordinate and follow up in a timely fashion.
  • Provide individual counseling.
  • Facilitate family conferences.
  • Assess needs and complete discharge planning when a participant dis-enrolls from Client.
Required Qualifications :

QUALIFICATIONS (knowledge, skills, abilities):


  • Graduate of an accredited college with a Master of Social Work degree.

  • Minimum one-year experience with the frail or elderly.

  • Bilingual Cantonese (preferred).

  • Experience with electronic health record system (preferred).

  • Minimum one-year post MSW experience (preferred).

  • Minimum one-year case management experience (preferred).

  • Minimum one-year health care related experience (preferred).



At this time, we can consider candidates that possess extensive case management experience.



Ideally, they should hold a bachelor’s in social work (BSW) and be currently pursuing their master’s degree.



Additional qualifications include a solid background working in a hospital or clinical setting. Language proficiency is a key component for this role; candidates must be fluent in Mandarin and Cantonese or, at a minimum, proficient in Cantonese and English.
