General Summary:
- Coordinates patient care services and resources to assure optimal clinical quality and financial outcomes
- Works collaboratively with Case Management Coordinator – RN and other clinical team members
Minimum Qualifications:
- Master’s Degree in Social Work, LCSW preferred
- Licensure/Certification/Registration: Current Maryland licensure as Social Worker required; Certification in case management desirable
- Experience in a clinical setting as a hospital discharge planner
Salary Range: Minimum 36.47 per hour - Maximum 60.17 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
