We are currently looking for a full-time (40 hours, Evening) Clinical Pharmacist at Newton-Wellesley Hospital. The ideal candidate will be responsible, trained, and competent for the interpretation, verification, order entry, compounding, and dispensing of pharmaceuticals and pharmaceutical supplies for adult, geriatric, and pediatric patients. The Clinical Pharmacist is also responsible for assuring the safe and effective use of medications, provision of drug information, and patient counseling. They will be responsible for assisting in the development and coordination of all the activities of pharmacy services.

Main Responsibilities

  1. Interprets and evaluates medication orders prior to entry/verification
  2. Clinically assesses orders for dose, route, frequency, and appropriateness of medication therapy
  3. Monitors orders for drug interactions, adverse reactions, incompatibilities, therapeutic duplication, allergies
  4. Identifies, mitigates, prevents, and documents problems with medication orders or therapeutic regimens, including medication errors or adverse drug reactions
  5. Verifies the accuracy of order entry and of medication products dispensed
  6. Delivers optimal pharmaceutical care by participating in direct patient-care interdisciplinary rounds
  7. Advises nurses, doctors, and patients on the proper and safe use of medications
  8. Narcotic control and assists in record maintenance
  9. Oversees pharmacy technician activities
  10. Assists in developing and implementing departmental clinical and educational programs
  11. Assists in inventory control and proper medication storage
  12. Compounds extemporaneous medications
  13. Prepares IV admixtures as needed. Inspects completed I.V. solutions for accuracy and compatibility
  14. Complies with USP 797 and USP 795 when compounding or overseeing compounded medications
  15. Prepares prescriptions in accordance with pharmacy laws and practice
  16. Maintains proper licensure and educational requirements and provides documentation upon request

Qualifications - External

  1. The ideal candidate will have a Bachelor of Science Degree in Pharmacy or Doctorate of Pharmacy Degree from an Accredited College/University of Pharmacy.
  2. Massachusetts Board of Registration in Pharmacy Licensure is required.
  3. 1 - 4 years clinical pharmacy experience in acute care hospital preferred
  4. Completion of ASHP accredited Pharmacy Residency preferred
  5. Board Certification (e.g., BCPS Board Certified Pharmacotherapy Specialist or other) preferred
  6. Knowledge of medication management of adult, and older adult patients required
