Travel Licensed Clinical Social Worker - $2,176 per week in Everett, WA

Job Title: Licensed Clinical Social Worker Profession: Social Services Specialty: LCSW Duration: 13 or 26 weeks with possible extension Shift: Day Hours per Shift: 8-hour (08:00 - 16:30) Experience: 1 year License: WA license required Certifications: MSW and BLS Must-Have: Discharge experience Description: In need of a Licensed Clinical Social Worker with discharge experience. This person will need to be an independent problem solver. A clinical social worker would be preferred. Must have complex discharge experience. Must work well with the formal discharge planner on site. Responsible for providing post-care planning with the patient. First-timers accepted with the right experience. Weekend work may be required. Floating as needed between discharge locations. Locals not accepted at this time within a 50-mile radius. RTO Restrictions: none, please submit all requests during application. Must work 2 of the 3 winter holidays. This is a new program in the state. This position will be part of the complex discharge pilot/enhanced care management. Preferred experience in guardianship, working with DHS, brokering with outpatient services, housing, and homelessness. Local travel is required, may need to go between various campuses and local shelters for patient care and follow-up. Average daily census is 15. Travelers are expected to float regularly between various units within their skill set. Travelers may be asked to float within the hospital or to a nearby sister hospital. Charting experience in EPIC required. Minimum of 2 supervisory references covering 1 year of employment within the last three years required. If required for the assignment, communication devices needed to perform duties will be supplied. Modules are non-billable with an average of 16 hours spent.
