The Geri PACT Director is assigned to the Geri PACT/Geriatrics Consultation Section of Geriatrics and Extended Care (GEC) Service, VA Pacific Islands Healthcare System (VAPIHCS). VA PIHCS provides patient centered care, delivered by engaged, collaborative teams in an integrated environment that supports learning and continuous improvement. The Geri PACT Director oversees both Geri-PACT and Geriatrics Consult clinics. The Geri-PACT Director must have outstanding clinical, interpersonal skills, organizational and administrative skills.

The Geri PACT Director is also a Geri PACT physician who can provide care in both Geri-PACT, a special population PACT providing care for complex, vulnerable at-risk Veterans, and Geriatric consultations in a consultant role. The Geri PACT physician works with the inter professional team to establish plan of care for Veterans, participates in inter-professional team meetings, appropriately facilitates utilization and access to other needed medical services, monitors quality and service metrics, provides education to staff as needed, teaches trainees rotating in Geriatrics in the outpatient clinic, documents appropriately in the electronic medical record, adheres to confidentiality practice, completes mandatory trainings. The Geri PACT physician participates in research as appropriate and education/training activities. The physician must meet requirements for academic appointment with the University of Hawaii John A. Burns School of Medicine, Department of Geriatric Medicine. The Geri PACT physician will be involved in the education and training of residents, fellows, medical students, and non-physician clinical trainees.

The Geri PACT Director additionally oversees the Geri-PACT and Geriatrics Consultation clinics and supervises the frontline team, including the Geri PACT Physicians, RNs, LPN, Health Tech, SW, and Medical Support Assistant. The Geri PACT Director works with the Chief of GEC and/or ACOS of GEC to establish and maintain appropriate level service agreements as needed, reviews workload information to balance clinic workload and report as needed, ensures overall panel capacity is maintained based on local patient complexity, ensure Geri PACT program meets standards for PACT programs, ensures that quality assurance program for Geri PACT encompasses both required PACT metrics and VHA quality indicators that target frail elderly, communicates mission and goals, outcomes, resource needs and recommended sources, urgent issues and options for address them, noteworthy accomplishments to staff and relevant clinical services on a regular basis including VA leadership as ACOS of GEC.

Supervisory Controls: The Geri PACT Director reports to the Chief of Geriatrics and Extended Care who reports to the ACOS of GEC.

Work Schedule: 8:00am - 4:30pm; Monday - Friday
