PRIMARY DUTIES AND RESPONSIBILITIES
- Assists Valley View inpatients and outpatients and their families in the overall management of their plan of care across the continuum.
- Provides psychosocial assessment and intervention for patients experiencing distress employing individual consultation, family, and group methods.
- Practices within the guidelines of professional social work ethics and standards.
- Completes a comprehensive assessment of patients including race, ethnicity, religion, culture, language, physical or mental disability, socioeconomic status, sexual orientation, and gender identity, and psychosocial status including their functional strengths, limitations, and deficits.
- Responsible for psychosocial support and brief counseling for family members of hospitalized patients and provides resource information and referrals as indicated.
- Links with RN Case Managers and completes discharge planning on complex discharges.
- Participates with multidisciplinary team and family to create a plan for post-acute transition of care.
- Develops and maintains contact with community agencies providing vital psychosocial services to Valley View Hospital's patients and attends hospital and community meetings as assigned.
- Engages in meetings, programs, and activities of the department.
- Participates in quality improvement initiatives.
- Documents according to department and hospital standards.
- Participates in orientation of new department staff, students, physicians, nurses, and volunteers.
SALARY RANGE:
- $29.05 to $42.73/hour + 14% PRN Differential Pay.
- Entry salary dependent upon education, skill set, and experience.
