Job Responsibilities
- Conduct Home Health Nursing assessments to determine the nursing, personal care, and equipment needs in the home, including preferences and goals of the participants. Participate in interdisciplinary team discussions when necessary.
- Regularly re-evaluate participant nursing needs and initiate the plan of care, making necessary revisions as participant status and needs change.
- Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions. Include the participant and the family in the planning process.
- Deliver and document home health nursing interventions as agreed upon in the participants' care plans, including maintaining a healthy and safe environment, responding promptly and accurately to physician orders, and correctly administering medications and performing ordered tests and treatments.
- Instruct the participant, family, and caregivers regarding the disease process, self-care techniques, and prevention strategies related to their medical conditions.
- Remain alert to pertinent input from other team members, participants, and caregivers, and update the interdisciplinary team promptly of any changes in participants' condition or medical status.
- Work with the Client Providers and other members of the interdisciplinary team to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
- Identify discharge planning needs as part of the care plan development and implement prior to discharge of the participant.
- Instruct, supervise, and evaluate home health aide care provided every two weeks.
- Participate in end-of-life care coordination and support.
- Provide backup coverage to other Client nursing functions.
- Perform duties consistent with the Nursing Practice Act, including the Standards of Competent Performance, Title 16, Chapter 14, Section 1443.5 of the California Code of Regulations.
- Work a varied schedule that may include evenings, nights, weekends, and overtime.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Follow Client policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
- Communicate regularly with Supervisor and team regarding workload and priorities.
- Complete all mandated trainings and education in a timely manner.
- Complete all mandated occupational health screenings as needed.
- Exercise flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
- Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
- Ability to initiate venous access for IV hydration or to draw labs; experienced with administering IM, SC, or transdermal injections, and providing wound care.
- Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
- Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.

- Minimum of two (2) years' nursing experience in a clinical setting with a frail or elderly population, home health agency experience preferred.
- Bilingual English/Spanish preferred.
- Graduate of an Accredited School of Nursing
- Bachelor of Science in Nursing (BSN) preferred.
- Unencumbered Registered Nurse (RN) License with which you practice required.
- A registered nurse shall meet qualifications established by the home health agency for the services provided and any additional qualifications required by home health agency licensure regulations.
