Description
The hospice Nurse Practitioner (NP) works in collaboration with Suburban Hospice staff to provide comprehensive, quality hospice services under the direction of the Medical Director. The NP will be responsible for the medical management of patients in the hospice program, ensuring that all aspects of patient care meet regulatory standards, and leading the clinical team to ensure coordinated care. The NP will fulfill the requirements of the face-to-face encounter for Medicare beneficiaries enrolled in Suburban Hospice.
The Nurse Practitioner provides medical support and coordination to hospice patients and family members. This includes duties such as: Provides medical expertise on pain, symptom management, and the evaluation of terminal prognosis to patient, family members and staff Ensures the care being provided is based on established hospice and palliative care guidelines and standards Acts as the attending physician for patients that request it Provides expertise and oversight of pharmacy formulary and medication expenditures Provides Suburban Hospice's Medical Director with face-to-face encounter documentation Provide education to staff, patients, and families on hospice care, pain management, and other relevant topics. Stays current on the latest medical practices, guidelines, and innovations in hospice and palliative care.

Requirements
Basic Qualifications Must be an MSN from an accredited nursing school with a current State of Indiana License as a Nurse Practitioner in good standing. 2-5 years' experience in hospice, palliative care or a related field is required. Knowledge of hospice care federal and state regulations. Knowledge of patient electronic medical record and obtaining patient histories Knowledge of clinical operations and procedures Ability to maintain quality, safety, and/or infection control standards Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in hospice and palliative care Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies Knowledge of related accreditation and certification requirements Knowledge of Medicare Face to Face encounter criteria Knowledge of the interdisciplinary team concept Ability to work collaboratively with referring, primary, consulting physicians and other members of the interdisciplinary team to provide appropriate care to patients Must possess valid driver's license and automobile insurance.
