Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (1 to 5 years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. Responsibilities This position is assigned to the Home-Based Primary Care (HBPC) Program. The HBPC Program provides comprehensive, interdisciplinary primary care services in the homes of veterans with complex and chronic, disabling disease. In addition to primary care interventions, HBPC provides palliative care, rehabilitation, disease management, and care coordination services. HBPC targets veterans in advanced stages of chronic disease, particularly those at high risk of recurrent hospitalization or nursing home placement. The Clinical Dietitian is the medical nutrition expert. At this grade level, the clinical dietitian is responsible for independently planning, developing and directing all nutritional care activities for assigned veterans, including those with complex medical and nutritional needs. The clinical dietitian supports the facility's mission and coordinates nutritional care with other health care providers, the veteran, the Veteran's caregiver and the community. The increasing complexity and specialization in nutrition requires that the clinical dietitian have expertise in a wide range of disease entities and treatment modalities. The clinical dietitian's expertise is specific to diagnosing nutrition problems and the nutrition ramifications of disease entities and medical treatment modalities. Assessment methodologies such as identifying physical signs of malnutrition, drug/nutrient interactions and depletions, and assessment of intake and nutrient losses are critical skills used by clinical dietitians. Duty Location (s): Assigned duty station of Honolulu, Hawaii. HBPC is considered mobile work involving travel between home visits and charting/working from home and/or in the office. Use of a government vehicle will be required, including to Veterans' homes and to the VA home care office(s). Travel to the neighbor islands will be required. Registered Dietitian Nutritionist (RDN) or Registered Dietitian (RD) - Must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA). No person will be appointed under authority of 38 U.S.C. chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English Current VHA Dietitians that qualify based on being covered by the Grandfathering Provision may not be promoted to a higher grade if they do not meet the current RDN/RD requirements. Grade Determinations: GS-11 Dietitian (Clinical) Experience. At least 1 year of experience at the next lower grade level (GS-09) that demonstrates the core competencies described at that level. In addition, the candidate must demonstrate the following technical KSAs : (a) Skill in delivering patient care using the NCP (e.g., assessment, diagnosis, intervention, monitoring and evaluation). (b) Skill in interpreting research to apply evidence-based nutrition practices to improve patient outcomes.(c) Skill in developing patient-centered goals. (d) Skill in analyzing data for performance improvement studies. (e) Skill in collaborating with stakeholders. (f) Ability to comply with various accrediting, regulatory and agency authorities. Physical Requirements: light to moderate physical activity including sitting, walking, bending, and carrying supplies such as papers, books, or small parts. Use of fingers, both hands required, ability for rapid mental and muscular coordination simultaneously, hearing (aid permitted), and clear distinctive speech are all required.
