Compensation: $105,000 - $115,000/year
Location: Queens, NY
Schedule: Monday to Friday (4 days remote, 1 day a week in the field)

Responsibilities as the MLTC Nurse Care Manager (RN):
- Care Coordination: Collaborate with members, families, and interdisciplinary teams to develop and implement personalized care plans.
- Comprehensive Assessments: Evaluate members' medical, social, and emotional needs to ensure holistic care coordination.
- Service Authorization: Approve services and coordinate care across payers, ensuring timely and appropriate interventions.
- Progress Monitoring: Track member progress, update care plans, and communicate changes to relevant stakeholders.
- Resource Management: Identify and connect members to community resources to support independence and well-being.
- Advocacy and Support: Serve as an advocate, ensuring members receive quality care and access to necessary services.
- Education: Bachelor of Science degree in Nursing required.
- Licensure: Active and unrestricted New York State RN License and registration.
- Experience: Minimum of 3 years of clinical RN experience, with expertise in geriatrics and managed long-term care.
- Skills: Strong organizational, communication, and problem-solving skills, with a commitment to providing compassionate care.
- Language: Bilingual skills may be required to serve diverse populations effectively.
