Overview Primary Care Practice Collaborating Physician: Dr. Arpan Desai - MTH 730A-4P - TW 730A to 6P - F 8A-4P or 730A-330 - Saturdays monthly 8A to 12PM Job Details A Nurse Practitioner (NP) at CHS (Catholic Health Services) is seeking a highly skilled healthcare professional with advanced nursing training who independently assesses, diagnoses, treats, and manages patients' health conditions across various clinical settings, including primary care, specialty clinics, and urgent care, by performing physical exams, ordering tests, prescribing medications, providing patient education, and collaborating with physicians to ensure optimal patient care, all within the scope of their state practice guidelines. Key responsibilities of a CHS Nurse Practitioner may include: Patient Assessment and Diagnosis: Taking comprehensive patient histories, including medical and social details. Conducting thorough physical examinations. Ordering and interpreting diagnostic tests like lab work and imaging studies. Formulating accurate diagnoses based on clinical findings. Treatment Planning and Implementation: Developing individualized treatment plans for acute and chronic conditions. Prescribing medications and managing medication regimens. Initiating therapeutic interventions including non-pharmacological treatments. Monitoring patient progress and making necessary adjustments to treatment plans. Patient Education and Counseling: Providing clear and comprehensive health information to patients regarding their conditions and treatment options. Educating patients on preventative health practices and lifestyle modifications. Addressing patient concerns and providing emotional support. Collaboration and Coordination: Collaborating with physicians and other healthcare providers to ensure coordinated patient care. Referring patients to specialists when necessary. Communicating effectively with patients, families, and other healthcare team members. Clinical Practice Management: Adhering to CHS policies and procedures, including documentation standards. Maintaining accurate and up-to-date patient records. Participating in quality improvement initiatives and clinical research activities. Required Qualifications: Master's degree in Nursing (MSN) from an accredited program with a Nurse Practitioner specialization. Current registered nurse (RN) license in the state of practice. National certification as a Nurse Practitioner (depending on specialty). Experience in clinical practice, depending on role. Salary Range USD $66.55 - USD $90.00 /Hr.
