Job Description
Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who see it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking full-time OB/GYN to join our team in Southampton.
Diagnosis and treatment of patients in his/her specialty area; collaboration and supervision of nurse practitioners and physician's assistants; participation in planning and developing health care services in his/her specialties, treatment of conditions and illnesses within the scope of the physician's specialty, referral and follow-up of patients when indicated, participation in after hours coverage to insure 24-hour availability of care for Center patients. Performs job duties in compliance with systems and job specific standards.
Essential Duties and Responsibilities: Daily Clinical Assessment: Takes Histories, examines, diagnoses and treats as per specialty scope of practice and privileges granted; Documents appropriately with specific information required to understand the scope and needs of the patient visit in both HPI and treatment plan = M Follows evidence based guidelines, US preventative task force recommendations and Chronic disease management guidelines with each visit and each patient; Sees every visit as an opportunity for preventative care and works with team to update all preventative measures needed with every visit; Aware of new guidelines recommendations, teaches team and implements them in practice; Completes Social and Family Hx. regularly on all patients = E Chart Documentation: Uses EMR for all documentation, including updated problem list and all interfaced labs = M Exceeds PCMH, e-scribing and Meaningful Use of expectations for providers over 85% of the time; develops new templates or order sets to improve workflow = E Referral, Communication and Multi-Disciplinary Approach in Management and treatment: Refers patients appropriately, including multi-specialty approach of care for specific and complex chronic care, as evidenced by peer review audits; exercises good judgment in determining when to seek professional expertise; i.e. Behavioral health, Pain management, Specialty Care, Nutrition Counseling, Uncontrolled diabetics; = M Uses Cognos reports to call patients in for visits; uses panel information to work with team and contact patients for f/u visits; works with team on chronic disease management; exceeds preventative care benchmarks = E Completes and Participates in Chart Review process: Completes peer audits as per SRH guidelines (30/year) = M Adds suggestions to improve value of audits and implements; Discusses audits with site medical director or provider, improves disease management from audit feedback; Review audits and incorporates suggestions into practice = E Displays Compassionate Patient Care and Sensitivity Displays cultural sensitivity, compassionate care and an understanding of patient needs, concerns and socioeconomic conditions, as evidenced by patient experience, feedback and surveys in CAHPS of at least 83% = M Over 85% CAHPS, Planetree award or other special state, regional specialty recognition = E Committed to continuing Education, Growth, and Expansion: Annual CME = M Attends annual CME, plus additional training webinars, conference or special training to increase skills; i.e eCW webinars, LGBT training, Essential Learning programs, Pain Management; works with team to incorporate new guidelines during patient visits; Enthusiastic Management; works with team to incorporate new guidelines during patient visits; about working on new projects, Americorps, research, new programs = E Attendance and Participation in clinical Provider Meetings:Attends or call in to monthly provider meetings (10), bi-annual provider meetings (1-2), monthly staff meetings (8-10); = M Attends SAM-QI meetings at least one a year; Call into Best Practices Call = E Professional and Respectful communication with Staff:Communicates in a respectful and professional Manner with staff, nurturing a team environment in the workplace; = M Acknowledges team regularly; works with team to improve their skills, teaches them, creates a patient-centered environment, involves team in patient care, values their commitment to their panel of patients; Excellent team feedback = E Minimum Education Requirement: Doctorate Degree (including MD, DO, DD, PhD, etc.)
Minimum Licensure: Board Certified and CPR/BLS
Required Skills & Knowledge: - Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers. - Competence in delivery of health care to patients (including preventative health education) - Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction. - Bilingual in both English and Spanish (orally and written) is preferred

Annualized Salary Range: $287,560.00 - $309,920.00
