Effective Immediately, you MUST include your New York Temporary Health Care Service Agency License/Registration Certificate up front with all Glens Falls Hospital nursing submissions. COVID vaccinations not required, however strongly encouraged. Average daily census: 20 patients RN to patient ratio: 1:9 (depending on census) Patient Population: Adult and Geriatric Average stay length: 5-7 days EMR: EPIC Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED! NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). Dress Code: Personal scrubs of any color - No designs or cartoons. No hoodies. Non-Billable Orientation: 8hrs Multipliers (OT, Holiday): 1.25% Job Requirements

This is a firm requirement. Clinician MUST be reliable! No attendance concerns, call outs, etc... - very patient oriented Unit Work History Skills Checklist (pertinent to position), current to 1 year 1 Supervisory Signed Reference (from within the past two years of employment) License/Certifications - Must be uploaded with profile (Current Nursys, BLS and CPI - Facility will not pend license/certs) Permanent address must be included in profile.
