Job Objective:
A brief overview of the position.
A professional Registered Nurse with specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process provides education and resources to facilitate informed decision making and timely access to quality health and psycho social care throughout all phases of the care continuum. Works with patients referred for assistance in accessing and utilizing health care resources due to their complex chronic illness.

Two (2) years experience in clinical setting.
Preferred: Two (2) years of experience in chronic disease management, case management, utilization management, and adult acute care
Essential Responsibilities
Assessment: Collects comprehensive data pertinent to the patient’s health or situation.
Diagnosis: Analyzes assessment data to determine the diagnoses or issues.
Outcomes Identification: Identifies expected outcomes for a plan individualized to the patient or the situation.
Planning: Develops a plan of care that prescribes strategies and alternatives to attain expected outcomes across the care continuum for inpatient and/or outpatient cardiac procedures.
Implementation: Implements identified plans.
Evaluation: Evaluates progress toward attainment of outcomes and revise as needed to meet goals.
Quality of Practice: Systematically enhances the quality and effectiveness of nursing practice using the Relationship Based Care model and shared governance for decision making.
Education: Attains knowledge and competency that reflects current evidence based practice for nursing care. Facilitates health and disease patient education, including leading group office visits. Supports patient self-management of disease and behavior modification interventions.
Professional Practice Evaluation: Evaluates personal nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations. Promotes clear communication among a care team and treating clinicians by ensuring awareness regarding patient care plans.
Collegiality: Interacts with and contributes to the professional development of peers and colleagues. Participates on a team for data collection, health care outcome reporting, clinical audits, and programmatic evaluation.
Collaboration: Collaborates with patient, family, and others in the conduct of nursing practice.
Ethics: Integrates ethical provisions in all areas of practice. Utilizes the ethic decision making model to drive issues and advocate for patient wishes.
Research: Integrates research findings into practice. Monitors the progress of patient through the service plan and re-evaluate the effectiveness and appropriateness of the plan based on the patient’s condition and needs.
Resource Utilization: Considers factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing services. Provides patients linkages to services to promote a pathway to care and positive outcomes.
Leadership: Provides leadership in the professional practice setting and the profession.
Practices within the scopes and standards of practice set forth by the American Nurses Association.
Practices within the scopes and standards set forth by professional organizations specific to specialty, if applicable.
Promotes development and ongoing success of the Remote Patient Monitoring (RPM) program.
Supports additional projects related to care coordination across the health system.
Performs other duties as assigned within area of expertise.
