    Job Requirements
  • American Heart Association BLS certification is required for clinical positions.
  • Primary Source Verification of professional license and/or certification is required.
  • Proof of COVID-19 vaccine series or completion and approval of client medical or religious exemption form is preferred, not required.
  • Positive MMR titers ONLY from within the last 15 years are required.
  • Fit Test must be completed within 1 year of start and updated annually.
  • Proof of full Hepatitis B series or at least two vaccines from the series, or signed declination, agency form acceptable.
  • If positive for PPD, a Chest X-ray within 5 years and current Questionnaire are needed.
  • Positive Varicella titers ONLY from within the last 15 years are required.
  • Proof of Tdap vaccination within past 10 years is mandatory, declinations not accepted.
  • ARRT Certification is required.
  • Annual Negative PPD is needed if a 2-step PPD not done within the past year.
  • UHS I-9 Attestation must be provided by the agency.
  • UHS Social Security Trace to be completed if not done with background check.
  • Ohio residents must meet fingerprint requirements with Ohio Fingerprints (BCI&I check) AND FCRA-Compliant National Database Criminal background check.
  • Highest level of education must be verified; if international, equivalency document from an accredited organization must be provided.
  • Proof of verification of employment within 5 years must be provided through third-party verification.
  • Competency Exams for the relevant department must be completed with at least an 80% score.
  • UHS Core Competencies must be completed with an 80% passing score.
  • Physical/Health Statement must indicate ability to work without restrictions.
  • Influenza Vaccine is required if assignment is from Oct-March, exemptions must be submitted for approval.
  • 2-Step PPD OR IGRA (Quantiferon/Tspot) with negative results within the last 6 months is required.
  • Resume to include Education and Work History, 2 Professional References within the last 2 years, and completed Job Application are necessary.
  • UHS Badge Photo is needed for identification.
  • UHS Criminal Background Check Results must be provided and ran within 6 months of Start.
  • UHS OIG Search is to be completed prior to start and is not an annual requirement.
  • UHS SAM/EPLS/GSA/OFAC screening to be completed and supporting documentation provided.
  • 10 panel UHS Drug Screen to be performed and if positive, include MRO report.
