The successful candidate will be responsible for coordinating and managing the comprehensive assessment process for residents, ensuring compliance with regulatory requirements, and developing care plans that address medical, functional, and psychological needs. Key responsibilities include: Coordinating the completion of Minimum Data Set (MDS) assessments for residents in accordance with federal, state, and local regulations Ensuring accuracy and timeliness of MDS documentation, care planning, and assessments for residents in skilled nursing facilities Collaborating with interdisciplinary teams to develop individualized care plans based on MDS findings Reviewing and analyzing clinical data to identify areas for improvement in patient care Requirements and Qualifications

To be considered for this role, applicants must have: A Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) A valid Registered Nurse (RN) license in the state of practice MDS Coordinator certification (ACM, RAC-CT, or CRRN) is preferred Compensation and Benefits

The pay package for this role includes a tax-free stipend amount, which will be determined based on 8-hour shifts and 40 hours per week. The estimated salary for this position is $
