*Key Responsibilities*: * Assess and treat patients in a Skilled Nursing Facility (SNF) setting * Develop individualized treatment plans based on patient needs and goals * Document progress and treatment outcomes * Collaborate with other healthcare professionals to optimize patient care * Provide education and support to patients and families
*Qualifications*: * Valid Occupational Therapist (OT) license in [State] * Experience in SNF therapy preferred but not required * Strong communication and interpersonal skills * Ability to work independently and as part of a team
