Job Description
UAS Assessment Nurse (RN) $85k - $105k + $15,000 Sign on Bonus Field-based job with various territories available including: ORANGE COUNTY, Staten Island, Queens, Manhattan, Brooklyn, Bronx, Westchester, Rockland, Duchess, Putnam, and Ulster counties. The position entails completing initial enrollment assessments and reassessments with a focus on clinical, psychological, and social needs of prospects and existing members using the Uniform Assessment System for UAS-NY in accordance with DOH-approved policies based on New York State regulations. Explains program rules and benefits of the plan in an easy-to-understand language. Reviews Member handbook, describes provider benefits and responsibilities, coordination of all covered services, and Medicare benefits as applicable. Conducts functional, psychosocial, and environmental safety assessments. Determines eligibility of enrollees. Establishes safe plans of care. Communicates plans of care to members, family, caregivers, healthcare professionals. Submits all assessment-related clinical documentation to ENPaperwork or NAPaperwork mailbox within established timeframes. All documents are submitted within 3 business days with an error rate of less than 5%. Qualifications: Associate's Degree in Nursing from an accredited school required, Baccalaureate Degree strongly preferred. A minimum of one year's experience in Community Health, Managed Care, or related field. Microsoft Word, Excel, UAS-NY certified. Estimated salary range is $100,000 - $120,000 per annum.
