Unit Information: Number of Beds -N/A Number of Staff – 22 Type of staff: Day Shift – Case Managers, Social Workers, Case Management Assistants Patient Ratios – 25:1 CM, SW 40:1 Type of equipment – MIDAS/CAREPORT/INTERQUAL EMR – MEDITECH Permanent Positions on Unit Available? (Y/N) – YES 3.5 FTEs Skill Set Most recruited for: (Travel RNs, local CNAs and sitters) Case Manager: Provides Case management service for both inpatient and observation patients assigned. Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care. Social Worker: Works collaboratively with RN case managers and the multidisciplinary team, performs assessments and develops appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs. Engages in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care. Additional Details
