The Case Manager, Justice Liaison Program works directly with individuals impacted by the criminal justice system, many of whom face challenges related to substance use, mental health, and systemic barriers. This role emphasizes trauma-informed care, case management, and connecting participants to critical community resources.
The ideal candidate will possess a deep understanding of the justice system through personal or professional experience and demonstrate empathy and compassion in their work.
Key Responsibilities
Participant Support and Case Management
- Conduct intakes and assessments to identify participant needs, including substance use treatment, mental health care, housing, and employment.
- Develop individualized case plans, setting achievable goals in collaboration with participants.
- Provide regular check-ins to monitor progress and address barriers to success.
- Offer crisis intervention and ongoing emotional support.
- Serve as a liaison between participants, probation officers, court personnel, and community service providers.
- Advocate for alternatives to incarceration, such as treatment programs or supervised release.
- Ensure participants comply with court mandates, attend hearings, and meet program requirements.
- Connect participants with local resources, including housing assistance, job training programs, counseling, and healthcare.
- Foster relationships with community organizations to expand resource availability.
- Co-facilitate workshops and group sessions focusing on life skills, emotional regulation, and personal growth.
- Maintain accurate, up-to-date case files and data records.
- Prepare timely court reports and compliance updates.
- Track participant outcomes to inform program improvements.
Required
- High school diploma or equivalent.
- Minimum of one year of experience in case management, behavioral health, or a related field.
- Familiarity with the justice system and its impact on individuals and families.
- Bachelor's degree in social work, psychology, or a related field.
- Lived experience with the justice system or personal recovery journey.
- Bilingual (English/Spanish) candidates are strongly encouraged to apply.
- Strong organizational and time management skills.
- Ability to communicate effectively with diverse populations.
- Crisis intervention and de-escalation techniques.
- Proficient in maintaining confidentiality and professional boundaries.
