Duties and Responsibilities: Working under the direct supervision of the Biological Scientist II, this position serves in a developmental capacity, providing direct disease and public health awareness intervention activities. This position is responsible for client intake, screening, and referral of persons referred under the (T & T) Test and Treat initiative who require linkage to HIV (H uman Immunodeficiency Viruses ) primary care and health support services. Conduct intake interviews and administers program screening instruments to identify range of client needs, barriers to care, and draft a care plan for clients. Ensure that clients are linked to community service providers within the established protocols. Provide partner services and health education to family members. Transport clients to and from T&T medical appointments as needed. Enters all necessary information on clients that are currently being linked to care in the database to ensure continuity of care. Conducts field investigations in accordance with current HIV Program Policy to locate and provide education and linkage to care to individuals newly infected with HIV/AIDS ( Acquired Immunodeficiency Syndrome) , or those that have been lost to care. Responds to phone calls from internal and external clients and customers requesting information about Linkage to Care Services and arrange intake appointments when appropriate. Coordinate client care with providers by telephone, as appropriate. Maintain client documentation in accordance with standard of care. Maintain accurate and confidential case records on services provided per Broward County Health Department procedures. Provides referrals to HIV-negative partners at highest risk of acquiring HIV (those in sero-discordant relationships) to biomedical intervention services, such as PrEP ( Pre-exposure prophylaxis) and nPEP ( Post-exposure prophylaxis ) . Provides partner services as required. Attend all appropriate meetings as required. Keep program Supervisor appraised of activities, challenges, and needs for assistance Perform other related duties as required. . Current employee of Florida Department of Health employees, (includes OPS, Career Service, SES or SMS employees). One (1) year or more experience working in the HIV field. Valid driver’s license, free of major infractions and access to an automobile to be utilized for work purpose. One (1) year or more experience working in STAR, Provide Enterprise and Linkage Module. PREFERED QUALIFITIONS: Minimum of one (1) year of experience transporting clients to Test and Treat appointments. Minimum of one (1) year of experience conducting field investigations with HIV clients. One (1) year experience with interviewing, counseling, case management, or public health.
