Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. POSITION SUMMARY The Hospice Aide provides personal care and assists in activities of daily living and related services to hospice patients. This employee functions under the direction, instruction and supervision of the Registered Nurse (RN) Case Manager. Care for the patient is given where the patient resides (i.e., patient's home, facility, hospital). We are looking for Aides in the following areas: Upper Butler County & New Castle area North Hills/Allison Park/Sewickley area QUALIFICATIONS Current Certified Nurse Aide (CNA) in the state of Pennsylvania preferred but not necessary Minimum of two [2] years experience in patient care preferred Ability to understand and carry out directions and instructions and maintain appropriate records RESPONSIBILITIES 1. Provides personal care according to the established individualized plan of care as developed by the RN member of the hospice team such as (but not limited to): bathing, grooming, dressing, shaving, feeding; checking vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation 2. Coordinates and communicates a weekly patient schedule with the RN Case Manager and Licensed Practical Nurse (LPN) to meet the optimal needs of the patient and family 3. Completes all computer and/or paper documentation requirements within the timeframes specified 4. Participates in patient and/or family teaching activities as directed by the RN hospice team member 5. Together with the hospice team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death 6.
