About the Role:

We are seeking a highly qualified Targeted Case Manager to join our team at our CCBHC. As a Targeted Case Manager, you will be responsible for providing comprehensive case management services to individuals with mental health and substance use disorders. You will work closely with clients to develop and implement individualized care plans, coordinate services, and monitor progress towards goals. Your work will be critical in helping clients achieve their desired outcomes and improve their overall quality of life.

Minimum Qualifications:
  • Bachelor's degree in social work, psychology, or related field
  • 2+ years of experience providing case management services to individuals with mental health and/or substance use disorders
  • Knowledge of evidence-based practices for the treatment of mental health and substance use disorders
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
Preferred Qualifications:
  • Master's degree in social work, psychology, or related field
  • Experience working in a Certified Community Behavioral Health Clinic (CCBHC)
  • Bilingual in English and Spanish
  • Experience working with diverse populations
Responsibilities:
  • Conduct comprehensive assessments to identify client needs and develop individualized care plans
  • Coordinate and monitor services provided by a multidisciplinary team of healthcare professionals
  • Provide ongoing support and advocacy to clients and their families
  • Maintain accurate and timely documentation of all client interactions and progress towards goals
  • Collaborate with community partners to ensure clients have access to necessary resources and services

Skills:

As a Targeted Case Manager, you will utilize your strong communication and interpersonal skills to build rapport with clients and their families. You will also need to be highly organized and detail-oriented to effectively manage multiple cases and maintain accurate documentation. Additionally, you will need to have a strong understanding of evidence-based practices for the treatment of mental health and substance use disorders, and be able to work collaboratively with a multidisciplinary team of healthcare professionals. Finally, proficiency in Microsoft Office and electronic health record systems is required to effectively manage client records and documentation.
